Role SummaryOur Home Health Aides serve a pivotal role in our efforts to provide compassionate, top-notch care to our patients in the comfort of their own homes. ResponsibilitiesSupports a patient's Plan of Care (POC) by monitoring and recording patient condition, providing support and personal services, and teaching familiesMonitors and assesses patient condition by observing physical and mental condition, intake and output, and exercise.
