Essential Functions:
- Supports, interprets, and implements Presbyterian Living’s policies.
- Works collaboratively with team to support Presbyterian Living’s person-centered care and Operation Excellence programs.
- Ensures adherence to all State and City regulations pertaining to Assisted Living.
- Maintains the documentation required for two state surveys.
- Participates in the inter-disciplinary service plan conferences.
- Supervises and directs the nursing staff, Resident Assistants, and any other nursing personnel assigned to Assisted Living.
- Assists in orienting and evaluating new nursing personnel and temporary personnel on assigned units.
- Assists with performance evaluations of staff.
- Counsels staff as needed and recommends disciplinary action to the Director of Assisted Living or initiates disciplinary action personally when the situation warrants.
- Maintains all documentation in the Wellness office.
- Coordinates resident medical appointments with physicians, transportation, and volunteers.
- Coordinates the Resident Assistant’s resident assignments adhering to the principles of person-centered care.
- Participates in direct patient care when staff is otherwise engaged, especially regarding medication administration, resident care, and emergencies.
- Enters pertinent resident information, including admissions, discharges, and condition changes, on the written 24-hour report and the 24-hour resident status sheet.
- Communicates changes in resident condition to the resident’s doctor in a timely fashion while taking appropriate action in urgent and emergent situations.
- Develops a collaborative relationship with representatives from other departments to coordinate person-centered care.
- Documents pertinent information on the inter-shift report and gives a verbal report to oncoming shift staff.
- Participates in monthly meetings to disseminate information and discuss departmental issues and concerns.
- Communicates with families to keep them informed of changes in patients’ conditions and/or needs.
- Monitors the quality and effectiveness of resident care in The Highlands on an ongoing basis and participates in QA studies as appropriate.
- Evaluates the performance of the nursing staff and Resident Assistants.
- Communicates to the Director of Assisted Living perceived developmental needs of staff.
- Encourages staff to actively participate in in-service and continuing education programs.
- Acts as a role model and resource person to all staff.
- Embraces the elements of the Engage program.
- Assumes responsibility for own professional development.
- Is aware of Presbyterian Living’s Corporate Compliance policies, including those related to confidentiality, security of information, and concern for the well-being of residents and patients.
- Demonstrates behaviors and completes work in a manner consistent with these policies.
Education: Graduation from an approved registered professional nursing program.
Qualifications:
- Active, current Illinois license to practice professional nursing.
- One year as an RN in a hospital (general care) or long-term care setting with adults preferred.
- Good communication skills.
- Interest in working with older adults.
- Ability to direct nursing assistants.
- Knowledge of aging changes and common health problems of geriatric patients.
- Current CPR certification required - accredited by American Heart Association.
