Purpose of the Job: Develop and implement the memory care Life Connections™ Activity Program to ensure social, physical, intellectual and recreational activities are available to enrich and engage the lives of our residents. Work closely with the resident care staff to communicate psychological and dementia issue changes.

Financial Responsibility: Moderate damage or loss is possible, but can be prevented by performing duties with considerable care and attention detail.

Responsibility for Contacts: Position involves contact with persons inside and outside the Company where resourcefulness, tact, confidentiality, and working knowledge of policies, procedures and practices are required.

Equipment: General office equipment: fax machine, copier, computer, telephone, calculator. Company Van.


Complexity of Work: Moderately complex work performed within established practices and procedures requiring original thinking.


Complexity of Supervision: Under direction as to assignments, general methods of execution with periodic review of assignments.



Responsibilities and Tasks:

Essential Job Responsibilities

1. Consistent and predictable attendance.

2. Admission Assessment: Get to know the resident during the first days to determine level of activity range to determine what activities the resident has the ability to participate in or complete. Communicate to nursing staff changes of residents likes and dislikes. Document activity changes. Give input into Care Plans by working with Wellness Director and family members/responsible party.

3. Monitor and intervene on psychological and dementia and communicate to resident care staff.

4. Train staff on dementia specific issues and programming including, but not limited to, Basic Training, Challenging Behaviors and Care for the Caregiver, ABC behavior and InStep™.

5. Help decorate the community for special events, seasons and holidays.

6. Serve as liaison with families and lead Family Programs such as Support Connections, Family Connections and Resources Connection.

7. Control expenses within approved budget.

8. Conduct community tours, back marketing duties and handle resident or family concerns in the absence of the Director of Resident Relations and/or the Executive Director as well as act as Manager on Duty on a regular rotation.

9. Relieve receptionist for lunch and breaks.

10. Transport assisted living and memory care residents to and from medical appointments, field trips, activities and other destinations in the Manorhouse van. Assist all passengers into and out of the van. Secure passengers with wheelchairs into seats with seatbelts. DO NOT TRANSPORT ANY PASSENGER IN A WHEELCHAIR. ALL PASSENGERS MUST BE IN A SEAT AND WEARING A SEATBELT.

11. In collaboration with the Home Office, Executive Director and Director of Resident Relations promote and maintain social media presence.

12. Develop and maintain a monthly resident and family newsletter which highlights activities, important information and visuals. Participate in the Activities Planning Team and then develop a fully functioning monthly programming calendar for the Pavilion.

13. Complete all Relias training as scheduled as well as program director meetings and required "dementia basics" training, program director orientation, train-the trainer and on-going training as required/provided.

Policy-Setting Responsibilities: Formally responsible for providing input into policies associated with the job's purpose and essential responsibilities.

Knowledge, Skills, Abilities or Information Required for Completely Satisfactory Performance:

  • Knowledge of principles for providing customer services including customer needs assessments and meeting quality standards for services.
  • Ability to give full attention to what residents are saying, taking time to understand, asking questions as appropriate and not interrupting.
  • Ability to train Pavilion employees on dementia and Life Connections™
  • Understanding and skill to maintain social media presence.
  • Ability to use third party vendor publishing software for newsletter.

Education and Licenses: Requires the ability to understand moderately complex mathematical formulas and charts or organize diversified information and apply fundamental principles of a science. Equivalent to a 4-year U.S. high school education and 1 year of advanced education or equivalent training in dementia recreation.

Experience: 1 to 3 years in dementia recreation.

Special Requirements:

  1. Must have ability to communicate as per section 22VAC40-73-110.
  2. Must be considerate and accepting of the aged, frail and disabled person.
  3. Appropriate attire and good personal hygiene are expected.
  4. Must comply with and implement all Company Policies and Procedures as well as the Employee Handbook.
  5. Must support and promote the "Rights and Responsibilities of Residents of Adult Care Residences."
  6. Must maintain confidentiality with regard to residents, staff and Company information.
  7. Must participate in training and in- service programs as required by the Company and/or applicable State's Standards including, but not limited to, OSHA required training, CPR, etc.
  8. Staff will not accept gratuities from resident, family members, vendors or visitors.
