The CCBHC is to ensure access to coordinated comprehensive behavioral health care. All CCBHCs are required to serve anyone who requests care for mental health or substance use, regardless of their ability to pay, place of residence, or age. This includes developmentally appropriate care for children and youth.The Child Center of NY is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at our Certified Community Behavioral Health Clinic (CCBHC). The nurse will play a key role in providing high-quality, patient-centered care to individuals experiencing mental health and substance use disorders. This position involves collaborating with an interdisciplinary team to promote wellness, administer treatments, and provide education on behavioral health conditions.Essential Functions:Assess clients need of a physical exam and conduct health assessments, including, but not limited to, communicable diseases, tobacco use, vital signs, medication reconciliation, and screenings for medical and behavioral health conditionsCoordinate medical care, including providing referrals to community medical providers, scheduling appointments, and liaising with medical providers to obtain patients' recordsConduct metabolic monitoring (i.e. blood pressure, weight and body mass index)Obtain and ensure processing patient lab work that occurs both internally and externally Administer medications, including injectable psychotropic medications, following prescriber orders, and organizational guidelines and safety protocolProvide basic first aid and manage medical emergencies and crisis situations appropriatelyMonitor and document patient responses to treatment and medications, reporting concerns to the prescribing psychiatric or medical provider and treatment team Provide patient education/counseling, both individually and in groups, regarding medications, chronic conditions, behavioral health diagnoses, healthy eating, exercise and fitness, family planning, safe sex practices, harm reduction strategies for substance use, and general wellnessEngage in outreach and follow-up to promote patient engagement and adherence to treatment plansSupport crisis intervention efforts, including de-escalation and collaboration with emergency services when necessaryCoordinate with pharmacies to ensure patients have timely access to prescribed treatmentsPerform and submit prior authorizations for medications, procedures, and other treatment services, including reviewing denials and obtaining additional information that could alter the initial decisionCollaborate with psychiatric providers, therapists, case managers, and other healthcare providers to coordinate comprehensive care Maintain accurate and timely documentation in the electronic health record (EHR)Participate in care team meetings, case consultations, and treatment planning discussionsEnsure compliance with state and federal regulations, as well as CCBHC quality and reporting standardsFulfilling other responsibilities that may emergeMinimum Qualifications:Graduate of an accredited nursing program (Associate's or Bachelor's degree in Nursing).Active LPN license in the state of New YorkTraining and experience working with people of all ages in behavioral health, mental health, or substance use treatment settingsCPR/BLS certification required Strong communication, problem-solving, and teamwork abilitiesAbility to work amongst diverse social, cultural, and economic groupsExcellent time management, phone, and face-to-face engagement skillsProficient in computer skillsQualifications Preferred:Familiarity with trauma-informed care, motivational interviewing, and crisis intervention techniques preferredLanguage other than English: Spanish, Mandarin, Cantonese, Bengali, Hindi, Korean, etc. Other languages may be required depending on client needs.Supervisory Responsibilities:None
