Schedule: Full Time Weekdays
Shift: Monday - Friday, 8am-5pm
Location: Servicing Gordon, Floyd, and Bartow.
As a Licensed Practical Nurse (LPN), you will be responsible for the delivery of coordinated nursing care to patients in their home environment, fostering a holistic, patient-centered approach. Under the guidance of a Registered Nurse (RN), you will oversee clinical activities, ensuring that each patient receives the highest quality care tailored to their individual needs.
Your essential responsibilities will include:
Providing optimal patient care and accurate documentation to uphold care standards and secure proper reimbursement.
Communicating effectively with other healthcare professionals for cohesive care and discharge planning.
Identifying educational opportunities for personal growth and demonstrating a commitment to professional development.
Setting and achieving professional goals with your supervisor to enhance performance and service delivery.
Understanding how personal and cultural values affect healthcare perceptions and practices to ensure quality care delivery.
Qualifications:
Practical Degree in Nursing
Basic Life Support AHA certification
Valid LPN License
Minimum of 1 year of experience in a hospital or community setting with direct nursing care
