Schedule: Full Time Weekdays Shift: Monday - Friday, 8am-5pm Location: Servicing Gordon, Floyd, and Bartow. As a Licensed Practical Nurse (LPN), you will be responsible for the delivery of coordinated nursing care to patients in their home environment, fostering a holistic, patient-centered approach. Under the guidance of a Registered Nurse (RN), you will oversee clinical activities, ensuring that each patient receives the highest quality care tailored to their individual needs. Your essential responsibilities will include: Providing optimal patient care and accurate documentation to uphold care standards and secure proper reimbursement. Communicating effectively with other healthcare professionals for cohesive care and discharge planning.
