The Opportunity
Join our High-Risk team as a Home Health Care Assistant, also known as a Field Care Coordinator. You will provide essential support to patients outside of the traditional clinic environment, visiting them in their homes following acute hospital stays and for routine care needs. In your role, you will facilitate telehealth visits, build trusting relationships, perform clinical assessments, and help patients navigate their care plans. Your work will be vital in connecting patients with community resources and teaching them how to use technology to coordinate their care.
What You'll Work On:
Facilitate telehealth visits in patients' homes, ensuring providers have a comprehensive view of the living situation.
Conduct patient assessments and document care plans to support effective communication between patients and providers.
Perform clinical duties such as health screenings, non-blood specimen collection, and basic vital assessments.
Coordinate care among patients, families, and external care teams while connecting them to community resources.
Assist in ensuring patient engagement with healthcare services and support in accessing necessary benefits and resources.
Contribute to daily operations within the High-Risk Care Team, including managing phone calls and facilitating transitions of care services.
Educate patients and families on how to utilize technology to connect with their care teams.

What You'll Need:
1+ years of experience in home-based healthcare or senior living settings.
Completion of an accredited Medical Assistant program or certification by a recognized organization.
Certified Medical Assistant (MA) and Basic Life Support (BLS) certification preferred, or the ability to obtain within 6 months of hire.
Excellent communication skills; Spanish-speaking is a plus.
Strong ability to build rapport with patients and deliver personalized care.
Preferred clinical skills in taking basic vitals and performing assessments.
Experience in connecting patients with community resources is desirable.
Proficiency with computer technology and video platforms is essential.
Access to reliable transportation for traveling to patients' homes; mileage reimbursed at the IRS/Federally established rate.
Compensation and Work Schedule:
The base pay for this position is $25.00 per hour based on a full-time schedule. This role is full-time, with shifts typically scheduled between 7:00 am and 6:00 pm, Monday through Friday, based in Phoenix, Arizona. You will need to travel throughout the Phoenix and Mesa metropolitan areas to visit patients in their homes.
