Position Summary: Home Health Aide / CNA
A Home Health Aide is a non-licensed member of the home care team who assists clients with daily living tasks as outlined in a care plan developed by the RN or Supervisor of Clinical Services. HHAs are supervised by the Supervisor of Clinical Services and receive scheduling from the office.
Qualifications:
At least 18 years old
Minimum one year of verifiable personal care experience (3 references required)
Completion of HHA training or CNA certification, plus a competency evaluation
Physically able to perform duties (lifting, transfers, ambulation)
Must read, write, and speak English fluently
Reliable transportation to and from assignments
Essential Functions:
Assigned to clients by an RN, with care instructions prepared and updated as needed
May perform duties within the scope of training and state law, including:
Personal grooming and hygiene (bathing, shaving, oral care, dressing)
Assisting with mobility, transfers, and exercises
Nutrition support, including feeding and hydration
Toileting and elimination assistance
Light housekeeping necessary for client safety
Medication assistance (self-administered only) including:
Retrieving the medication container
Confirming it is prescribed for the client
Reminding to take medication
Observing client self-administering
Other activities as taught and delegated by an RN per Connecticut law and agency policy
Restrictions:
May not change sterile dressings, administer medications, perform catheterizations, apply heat, or conduct procedures not in the care plan
All HHAs must complete agency orientation and pass documented competency assessments by a qualified RN.
