Responsibilities:
Case Management
Intake/enrollment of children
Assess strengths and needs of families, facilitate referrals and follow-up of referrals
Assist families with family goals setting and follow-up
Accompany parents to meetings as interpreters as needed
Work with families of children with special needs; attend case conferences & follow-up mtg.
Maintain complete, confidential and up to date documentation of family files
Maintain/update children’s health files along with the Nurse Consultant
Inform families about community resources and how to access services
Social Service Database System/Salesforce
Maintain and monitor database, ensuring all information is kept current
Determine eligibility and follow up on recertification for Child Care
Maintain and update the waiting list
General Program Assistance
Assist with parent involvement activities, including meetings, workshops and special events
Assist with the implementation of the program evaluation process
Assist with the outreach and recruitment of children
Attend meetings, trainings or workshops off or on site
Assist in the classrooms and conduct home visits
Perform general office duties
Perform other duties as assigned by supervisor
Mental Health Services Support
Assist or conduct playgroups
Attend Mental Health Team Meeting
Make decisions regarding day-to-day operations in conjunction with the Director
Qualifications:
Bachelor’s Degree in Social Sciences or related field required
Experience working with children and families
Bilingual English/Spanish preferred
Must have excellent computer skills with knowledge of Microsoft Excel, Access, and Word
Work Schedule: Full time, Monday - Friday, 9:00 am - 5:00 pm, 35 hours a week
