POSITION SUMMARY:
This position will be able to work in all areas of the CARE program to support staff and management with day-to-day operations of running the program. This position will be responsible for the staffing grid to ensure coverage of internal programing and community activities. When needed, this position will provide coverage in various areas due to general staff call-offs or vacation time. This position will also support DSP’s to deliver quality therapeutic programming, and meaningful community experiences.
EDUCATION: Minimum of High school diploma or GED.
EXPERIENCE:
- Requires a minimum of one year of experience working in the DD field service delivery. Preference will be given to applicants with more experience.
- Must be able to pass criminal background investigation and various registry checks.
- Must pass DMV check, pass road test, DOT physical and pre-employment drug and alcohol test and random testing throughout employment.
- Knowledge of proper Behavior Support principles and Crisis Prevention Intervention.
- Must be able to compute and record data for participant documentation.
- Must be able to clearly and accurately communicate comprehensive information about client worksite behaviors to staff.
- Must possess superior judgment skills in order to ensure the health, safety and well-being of adults with developmental disabilities in independent settings.
- Provide coverage for Direct Support Professionals as needed due to planned or unplanned time off.
- Create monthly staffing grid to ensure efficient coverage and quality of program operations.
- Help facilitate internal programming activities and skills training
- Develop monthly community integration calendar based on participant strengths and interest
- Create weekly community integration sign up sheet from the monthly calendar
- Monitor and update Real Time Data tracking sheet to ensure participant desired community engagement opportunities are fulfilled.
- Provide DSP’s with topic subject matter for morning interactive activity.
- Maintain CARE daily calendar
- Revise & delegate internal programming, PCA schedule and community outings as needed
- Assist in pro-active disability management and non-violent crisis intervention as needed.
- Ensure the health and safety of the individuals served in a variety of activities and settings both in the community and the CARE program floor.
- Assist with training of new DSP’s to ensure adherence to health and safety of participants and quality programming standards of CARE program.
- Provide physical assistance with consumer's personal care needs such as toileting, changing, feeding and related tasks as needed.
- Other duties as assigned
- Assist in non-violent crisis intervention as needed.
- Complete annual trainings and attend agency meetings when feasible.
- Must be able to move around the facility and the larger community.
- Must be able to complete necessary digital and physical documentation.
- Full Time 40-hour work week.
- Must be able to meet the physical demands of providing personal assistance to consumers which entails frequent bending, stooping, reaching and lifting up to 35 lbs.
