Community Health Nurse (Fee For Service) at MJHS summary: As a Community Health Nurse (Fee For Service), you provide advanced clinical home care, including patient assessments, medication management, and education, supporting patients recovering from illness, injury, or surgery at home. You coordinate and evaluate individualized care plans, work closely with family and professional caregivers, and ensure quality and compassionate care within a large not-for-profit health system in Greater New York. This role requires strong clinical insight, decision-making skills, and nursing licensure with experience in acute medical-surgical and home health care settings. Fee for Service rates: Initial & ROC Weekday visit rate: $100 Inital & ROC Weekend/Holiday visit rate: $120 Revisit daily rate: $85 Orientation daily rate: $250 In-Service hourly rate: $55 Weekday and weekend shifts available.
