Empower and support each other.ResponsibilitiesThe Collaborating Physician works with an APN (or more than one) who is performing in home and/or telehealth wellness assessments on patients’ designated by HarmonyCares Medical Group. APNs working with the Collaborating Physician will not have prescriptive authority, nor will they be treating patients. As a Collaborating Physician you will:Be available via telecommunication or in-person for consultation with APN to address patient assessment questionsRegularly performs annual chart reviewsConsults with APN ensure adherence to policies, procedures, state laws and qualityBe accountable for outstanding customer service to all external and internal customersQualificationsActive/unrestricted Medical Physician LicenseBoard certified/board eligible requiredResidency completion required1 year experience in Healthcare managementExperience with Risk AdjustmentMust reside in the state where the openings existsPay TransparencyIndividual compensation packages are based on various factors unique to each candidate, including skill set, experience, qualifications, and other job-related considerations.NoticeHarmonyCares and HarmonyCares Hospice are not affiliated with Harmony Hospice Care.
