* Under the direction of Director of Care Management, this position demonstrates knowledge of biopsychosocial factors related to the ages of the patient's served and incorporates knowledge base when developing treatment plan .Coordinates the care and services of patient populations across the continuum of illness; promotes effective utilization and monitoring of health care resources; assumes a leadership role within the interdisciplinary team to achieve optimal clinical and resource outcomes; facilitates and coordinates the discharge planning process. * Education, Knowledge, Skills and Abilities * BA in social work or related field. Must be able to work independently and as part of a multi-disciplinary team. Crisis Intervention skills, Therapeutic communication skills.
* Required Length and Type of Experience * Two years of previous experience in a hospital or healthcare setting preferred.
* Licensure or Certification Preferred * LSW or working toward a degree in SW
* Required Physical and Environmental Demands * Requires direct patient contact. Required to walk, stand, bend and sit. Routine and/or daily tasks or procedures occasionally expose them to blood and other potentially infectious materials. Must be able to remain calm and function clearly in a stressful potentially violent environment.
