The Case Manager plays a critical role in coordinating care and services for individuals with mental health challenges. Working as part of a multidisciplinary team, the Case Manager assesses client needs, develops individualized service plans, and connects clients to appropriate community resources. The goal is to promote stability, recovery, and independence in clients’ lives through consistent support, advocacy, and service coordination. Behavioral Health Case Manager NSPIRE High school diploma/GED and three (3) years of job-related experience in a social services-related position; or one (1) year of experience at the lower level Social Svcs Tech 2 (SST011) or equivalent position.
