This part-time per week role is ideal for a provider who is passionate about geriatrics and values quality time with patients over quantity.

What Youll Do
Provide compassionate, relationship-based care to older adults in an assisted living environment.
Build trust with patients, families, and facility staff through clear communication and consistent presence.
Manage chronic conditions, address acute concerns, and tailor care plans to the unique goals of each patient.
Collaborate with the facilitys care team to ensure seamless, coordinated support.
Apply your home health or hospice experience to improve outcomes for medically complex patients.
Maintain flexible availability for patient visits and on-site presence.

Qualifications
Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in Georgia (unrestricted, non-probationary).
Masters degree from an accredited program.
Minimum of 3 years of clinical experience, with strong preference for geriatric, hospice, or home health background.
Active DEA license (or eligibility to obtain).
Availability to work on-site in Sugar Hill; occasional visits to Metro Atlanta locations may be required.
